Probably a combination of a lot of reasons.
Dirty, Japanese really like cleanliness so lots of girls ban it without ever trying.
Force, lots of JAV is rather forceful which is dangerous for anal, prepwork is done off camera I imagine for most
Crazy, JAV tends to go all out, very few are just anal if they have the anal tag it usually comes with enemas or BDSM or piss. Since they dont see it as a normal act it gets connected to way more extreme things in their titles?

Yukino Matsui has done some outstanding work. Not keen on her extreme stuff, but her anal work is awesome.
