半中半外. Does it mean “half in half out?” Is it an idiom?

So many halves . . .

半中 Han naka mid-half
半外 han soto half outside
半彼女 han kanojo half girlfriend

And if the cover says Han naka, why does google translate say 半中 is Han chū?

could be half-Chinese. 中 is sometimes short for 中国 “China”

so half-Chinese, half-foreign (if so, how is she even Japanese lmao)
